FACULTY CAREER DEVELOPMENT SURVEY

Survey of Assistant and Associate Professors

You are being invited to voluntarily participate in the above-titled research study. The purpose of the study is to document the career development needs of Assistant and Associate Professors (tenure, clinical and research tracks) in order to inform the development of future faculty development programs. You are eligible to participate because you are currently employed as an Assistant or Associate Professor (clinical, research or tenure track) in the College of Medicine or College of Public Health.

If you agree to participate, your participation will involve completing an anonymous on-line survey which will take about 10 minutes. If you prefer to complete a paper and pencil survey, you can down load a .pdf file of the survey and send it via campus of US mail it to the address on the survey: You may choose not to answer some or all of the questions.

Any questions you have will be answered and you may withdraw from the study at any time. There are no known risks from your participation and no direct benefit from your participation is expected. There is no cost to you except for your time and you will not be compensated for your participation.

This is an anonymous survey, so no one will be able to identify your answers to the questions. Only the principal investigator and members of the GRACE Committee (Generating Respect for All in a Climate of Excellence) will have access to the data.

By participating in the interview(s), you are giving permission for the investigator to use your information for research purposes.
